When applying for a green card in the United States, Form I-693, the Report of Medical Examination and Vaccination Record, is an essential requirement. This form, completed by a certified civil surgeon, establishes that you are not inadmissible based on health-related grounds. Here is what you need to know about the process, recent updates, and how Jarrett & Price can guide you through it.
What is Form I-693?
Form I-693 documents the results of a medical examination to determine if an applicant has health conditions that could affect their eligibility for immigration. It covers:
- Screening for communicable diseases.
- Assessing physical or mental disorders that may pose a threat to others.
- Ensuring required vaccinations are up-to-date.
Recent Changes: No Expiration for Certain Forms
As of November 1, 2023, Form I-693 signed by a civil surgeon no longer expires, provided it accurately reflects the applicant’s medical condition at the time of review. This is a significant update, offering applicants greater flexibility when filing immigration applications.
However, if there is reason to believe the applicant’s medical condition has changed or the form does not accurately represent their health, USCIS officers may request additional evidence or an updated Form I-693.
Key Requirements for the Medical Examination
- Authorized Civil Surgeon: Only doctors listed on the USCIS-approved civil surgeon database are authorized to complete the medical exam.
- Preparation: Bring the following to your appointment:
- Government-issued photo ID.
- Vaccination records (including COVID-19 proof, if applicable).
- List of current medications.
- Any relevant medical records.
- Exam Details: The civil surgeon will conduct health screenings, administer any required vaccinations, and sign the completed Form I-693. The form is then sealed in an envelope for submission to USCIS.
Important Submission Guidelines
Do not open the sealed envelope provided by the civil surgeon. USCIS will only accept the form if it remains sealed and unaltered. You’ll also receive a copy for your personal records.
Submission Deadlines
Beginning December 2, 2024, the completed Form I-693 must accompany your Form I-485 (green card application). Failure to include it could result in rejection of your application.
